1. Who is the data controller

Data submitted through this website is processed by IAMPERFORMANCE S.R.L., CUI 53055651, Trade Register registration number J2025094671000, with its registered office at Int. Gheorghe Simionescu nr. 19, ap. B26, Sector 1, București, represented by Mădălin Aguciu, physiotherapist.

2. What data we collect

Through the booking form, we collect only the information you provide: your full name, telephone number and preferred location. The form does not collect medical information; this is discussed and documented during your assessment at the clinic.

3. Why we use it and our legal basis

We use your data solely to contact you and arrange an appointment. The legal basis is your consent (Article 6(1)(a) of Regulation (EU) 2016/679 – GDPR), given by ticking the box on the form, and taking steps at your request before entering into a working relationship (Article 6(1)(b)).

We do not use your data for advertising and do not sell it to anyone.

4. How long we keep it

If we do not begin working together, we delete your form data within 6 months of the last contact. If you become a patient, your data is managed by the clinic where treatment takes place, in accordance with applicable legal obligations.

5. Who else has access

Only the technical providers we need to operate this website: the website hosting service and the service used to send the request by email. They process data only on our behalf and for this purpose.

6. Your rights

You have the right to request access to your data, its rectification or erasure, restriction of processing and data portability, and to object to processing. You may withdraw your consent at any time, without affecting processing carried out before withdrawal. To exercise any of these rights, email madalin.aguciu@gmail.com.

If you believe your rights have not been respected, you may lodge a complaint with the Romanian National Supervisory Authority for Personal Data Processing (www.dataprotection.ro).

7. Cookies

This website does not use marketing or tracking cookies. If we add traffic analysis tools, we will update this policy and request your consent where required by law.
